If you have Business/Developer versions of the plugin, you can copy the plugin folder and its databases, and you'll have the same settings/forms on the other site.

Please be informed, that if you copy the way mentioned in the previous post, the CSS should be copied. If it some way won't, please feel free to add a new theme on the new website and copy/paste the CSS of the desired theme.

Please note, that copying with FTP only will not copy the settings, styles, etc. So please log in to the MySQL database of your site (with PHPMyAdmin) and copy all the tables which have contactformmaker keyword in their titles to the other sites. They should be the following tables:

[table_prefix]_wp39_contactformmaker_blocked
[table_prefix]_wp39_contactformmaker_submits
[table_prefix]_wp39_contactformmaker_themes
[table_prefix]_wp39_contactformmaker_views
