TOPIC: Contact form showing disabled fields

Jennifer Winfree OFFLINE Fresh Boarder Posts: 1	Have started using Contact Form Builder for various websites and appreciated the ease of use. But after updating one form recently, it is now displaying disabled fields below the form. How can this be fixed? TIA Page in question: www.pci-insurances.com/contact-us/ Am using Wordpress 4.0
