How can I create a new field in the form?

I want to show two fields of checkboxes, but in Contact Form 1 there is one field of checkboxes.

May anyone help me?

Dear customer,

You can not add new fields with Joomla Contact Form Maker. You are only allowed to choose from those which are in your form.
