I have to put a fresh Wordpress install to remove all of the demo files created by ShopDock - My assistant has created 13 complicated forms. Will I lose my forms if I install Wordpress to start over?
This is a job I would like to continue tonight - if anyone has an answer I would appreciate solid - accurate answer.

It took hours to complete our forms - and I have too much junk because I imported the demo.xml

Dear Debra,

Please follow the steps below to migrate your current wordpress forms to the new one.

On your current Wordpress site:
1. go to phpMyAdmin
2. open your Wordpress database
3. select the Export menu item
4. change the radio button from Quick to Custom
5. unselect all tables and select the following 4 tables:
[wp1_prefix]_formmaker
[wp1_prefix]_formmaker_submits
[wp1_prefix]_formmaker_themes
[wp1_prefix]_formmaker_views
(where [wp1_prefix] is your Wordpress database prefix)
6. scroll all the way down and press the Go button. This will export an SQL file containing those 4 tables.

Install the fresh copy of wordpress and follow the steps below.
On your fresh copy of Wordpress site:
7. go to your Wordpress database
8. select the Import menu item
9. browse for the exported SQL file
10. scroll all the way down and press the Go button. This will import the form maker tables to your Wordpress database.
-- now you need to change the prefix for those tables to the one that is set for your second Wordpress database.
11. in the phpMyAdmin from the left pane of your new Wordpress database select the [wp1_prefix]_formmaker table.
12. select the Operations menu item from the top menu
13. scroll to the Table options box and edit the value of Rename table to field with the new Wordpress database prefix, so that the table name now looks like this [wp2_prefix]_formmaker (where [wp2_prefix] is your fresh Wordpress database prefix)
14. do the same for other 3 tables.
15. go to your new Wordpress administration panel and install the Form Maker.

That's it! Now you should have your forms, submissions and themes all set up.

Thanks so much. I struggled a bit - after the fresh WP install - because in haste I installed the plugin before I should have. So had to go into the database and delete those newly created tables. Then I imported the forms.sql from the old set up and POOF - there were my forms.
This has taken hours - but I am pleased.

Now - I need a little tweaking and I will be on my way -
Each form has a field that is currently 200px - there are 20 of them and I need to customize this field.
I need to keep it on the same line - but break the 200px single entry field into 5 small blocks that will accept two-digits each.
Also need a drop down for State/Providence - the country one is splendid.
One more thing - I deal with customers all over the world - is there a customization for phone number (country code - area code - etc) that will accommodate customers without manually setting these spaces)

I can't tell you how glad I am that these forms did not require being re-created.
