Load Form into Joomla Module

Is it possible to load a form maker form into a joomla module? Is there a work around for this? My client does not want the form to be accessible from a menu item. He wants it to stand out and be available through a module. I can't seem to accomplish this.

Thank you for your timely reply.
The Form Maker module works except that I do not want the form displayed with that module. I want it to link to a different page. By using the instructions you sent, the form prints over top of the other information on the page. See attachment photo.

This is the page I would like the module to go to:

The form works great by adding it to an article then making a link to it from the menu. However, the client does not want it in the menu. He wants it to stand out by putting the link in a module where it can have the blue heading and stand out more. So I made a link using a "custom HTML" module and it works. But because he does not want a link from the menu, just a link from the module, I turned off the menu item and end up with a 404 error. But as you saw with the image I sent last time, if I add a Form Maker Module, there is no real 'link' to another page. It just prints the form over whatever is on the page.
I will give you access if you will tell me where to send the information.
