All of a sudden all of the forms on my site are blank. I use several, maybe as many as 5

Two examples:

www.phoenixmag.com/customer-service/contact-support.html

www.phoenixmag.com/customer-service/subscribe.html

I can't figure out why. I haven't changed anything, although we have had some unrelated database issues , so I was forced to optimize it.
