Cannot access captcha or submit buttons

I installed the latest version of form maker to a website but while creating a form, the icons for creating the form are so large that I cannot see the captcha or submit buttons. I cannot complete the creation of the form.
Using joomla 2.5 and form maker 3.5.9

Dear Ruth,

Looks like the problem is perhaps browser-specific. We tested with Chrome and Firefox on Windows OS but form elements appeared as expected.

Could you please let us know what OS do you have? It seems that you are using Firefox as your web browser.

Dear Ruth,

Please check the screenshot attached. It looks like the problem is not seen in Microsoft Edge on Windows 10 either. Could you please try with Chrome and let us know if it works?
