Multi-Page Form = All pages display at once

I used the “Product Survey Form” sample form and did a “Save As Copy” and then renamed. The form has 3 built-in pages and I started modifying and adding new fields. When I view it on either a desktop, tablet, or phone, all 3 pages show up on the same screen (See attached screenshot). The different pages have their designated areas and there are the “Next” and “Previous” buttons displayed at the bottom of what would be each page.
TROUBLESHOOTING
I loaded the unmodified “Product Survey” form and it displays the same way. I am using the full paid version of Form maker in Joomla 2.5 with an Artisteer template.
When I switch to the default Joomla template then it works as designed. Obviously there is a conflict with my Artisteer 4.0 template. Do you have any ideas how I can over-ride this issue? I tried it with both the responsive and non-responsive Artisteer form options and there is no difference. Can you think of what may be causing the conflict? I really want to take advantage of the multi-page option to break up my long online order form.
I will go ahead and work on the form as though there is a solution.

The tech support team visited my site and found a conflict with my Artisteer theme and the multi-page form is now working. I am not sure what they tweaked, but they have assured me that all will work well in the future.
