Whenever I fill out the form and try to upload .docx file the entire site crashes and I get the following error "ERR_EMPTY_RESPONSE". The form and database work when using .doc or .txt files, please help I need this fixed immediately. Thank you.

I have searched the forums for this same problem and have found no response.

I have added .docx to the allowed upload filetypes and it is separated by commas as required.

Andrew, maybe the docx file is very large and that gives an issue.

If so, then please try to enlarge max_upload_size and post_max_size from php.ini file.
