Please use Custom HTML field to insert the Terms & Conditions link or text. Afterwards insert a checkbox with one option, which indicates that the submitter agrees with these terms.

Make sure to set the checkbox field required.

Unfortunately the checkbox cannot be placed in Custom HTML field. And as you noticed, these two can be separate entries only.
