Hi!

My form will not submit when I use "File Upload" in the form. When I removed it the form worked just fine.
Here is my form without "File Upload"

and here is with

What am I doing wrong?

Looks like there are Javascript errors thrown on both of the forms. So the problem is most probably a conflict with your website theme.
