All the fields in my form are appearing fine on my android but the captcha field shows a "?" instead of a code...and my husband's blackberry shows a red X instead of a code.

Hi again,
I have found the conflict that was causing the captcha issue with the form maker and it was a conflict with the Better WP Security plugin, Security Tab, uncheck "Filter suspicious query strings".
