Similar problem to others have had - I however have to add the shortcode via template files and its not rendering..besides the standard [Contact_Form_Builder id="3"] type of shortcode isn't there a php wrap option as well? tried to add it using the typical nomenclature but no dice.

I'm attaching the template file for that page..hope maybe that will help. Oh and, disabled all other plugins to make sure no conflicts..didn't help. Thanks!
