I have tried to follow every single step as described in the manual.
However, the most essential in this case is missing. The insert button in posts or pages.

The plugin is activated and the management and editing works fine.
Any idea?

Please update the plugin to the latest version and your issue should be fixed. It is a conflict with WordPress latest update. we have already fixed that issue.

I have wordpress 3.9.2

Cleared cached files , didn't help either.

I am having the same issue, I am guessing the answer is something straight forward, but I also do not have the contact form button and I have the most recent version of the contact form and flushed my cache.

It seems, that you have deactivated Contact Form Builder, since it is not in the side menu. Could you please check this in the Plugins section of the admin?
