I really like this plugin, however, the area code box isn't lining up correctly with the rest of the phone number. Also, the Address block is lining up with the other text boxes above and below it.

Any ideas on how to fix this?

Could you please tell us what browser do you use to view the form? We've tested on Google Chrome and Firefox on Windows OS and the fields looked as they should.

Only the address field is not lined up, perhaps because the label position is set to Top. Please set it to Left by editing the field.
