When i purchase this front-end form (i.e form maker), i was guaranteed a six month support. i want to know if the form will stop working (or submission stop) after the expiration of the six month support.

Please note, that you can use Form Maker for as long as you will need it. The support for the plugin after subscription period is over includes bug fixes. So in case something will break, we will take a look at it, surely.
