I would like to use form maker to accept information which then is stored in a searchable database, and is directed to a template that would create a page in wordpress with the information from the form, including jpg files.

Is this possible? If so, what database should be used?

Unfortunately Form Maker for Drupal does not have the mentioned functionality by default. The module works on MySQL database, so you can generate and run queries to show the submissions from the database on your website frontend.
