Limit number of check boxes that can be selected

I want to limit visitors to only being able to choose 5 selections from a list of many more. Is that possible? Where and how would I do this?

Please give us (to [email protected]) a link to your site and temporary admin user account, and we will try to find out the problem of WordPress Form Maker as soon as possible, also we will create custom javascript function that will limit the selections.
