When I add more than one form to a page, I get a ton of little, clickable squares next to all the forms except for the last one I placed. Any reason why I could not use multiples on the same page? Ever heard of this before?

I'm trying to allow voting on photos for a contest. I added a simple checkbox form for the bottom three photos. The bottom two have the little, clickable boxes but the third one from the bottom does not.

Also, am getting a stack overflow error in IE 8

Any help would be greatly appreciated.

Unfortunately currently WordPress Form Maker does not allow you to put more then one form on the same page. All the described problems come from this.
