When i try to upload the zip, it shows this:

File is empty. Please upload something more substantial. This error could also be caused by uploads being disabled in your php.ini or by post_max_size being defined as smaller than upload_max_filesize in php.ini.

It seems to be empty . . .

Dear Monique,
Please check your php configuration and make sure that file uploads are not disabled and upload_max_filesize is greater then 1 Mb.

This problem is most probably not connected with WordPress Form Maker, but if you give us a temporary admin user account to your site, we will try to find out the problem.
