Basically, I want to replicate whats been done on the Reg form demo.

How do we create the "Do you want additional information?" slider button, and how do we make that button reveal the hidden form fields? Are there limits to how many fields can be displayed in this way?
Essentially I want to create a form that based on one choice (are you blue or red), several form fields are displayed below. Of course these fields would be different depending on the choice, but still be displayed in the same location.
Is this possible?
