How can I embed an image into PDF when using Form Maker PDF Integration?
I see it in visualization, but when the pdf arrives it has the url but not the image

Thank you for letting us know about this problem!
We will update the add-on and include this fix. But here's a code you can use to make the corrections meanwhile. Please open form-maker-pdf-integration/dompdf/dompdf_config.custom.inc.php file find the following code:
define("DOMPDF_ENABLE_REMOTE", true);
It is currently commented, please uncomment it, as it is shown in the screenshot below.
Afterwards you will need to insert the image with the following format in Text mode of the editor in PDF custom text:
<img src="wp-content/uploads/image-folder/image-title.png" width="150" height="150" />
You will notice the source does not include the domain address (root), but starts with wp-content/ directory. Also feel free to alter the values of width and height as necessary.
