How do I set a minimum required quantity for my product?

For instance, a min of 5 pieces is required for each purchase, I do not want my customer to enter anything less than 5.

Unfortunately Form Maker does not have this functionality yet. But perhaps you can add a custom HTML field and notify the users, that the minimum number of items is 5.

Unfortunately the only way do accomplish this is to add enough number of required fields, which will all have values that add up to the minimum total value you wish to have.

Otherwise you might want to use Amount field of PayPal tools, which will let you specify a Range for the payment.
