Can someone please help me with the following. I want to create a multiple choice question for

Screens: front door
back door
patio door
window(s)

but for each of the above I would like to have radio buttons as follows:

front door - none, mosquito netting, proper screens
back door - none, mosquito netting, proper screens
patio door - none, mosquito netting, proper screens
window(s) - none, mosquito netting, proper screens

Dear Mary,

Unfortunately there are no options to create a hierarchy of checkboxes and radio buttons.

But what you can do to achieve a similar functionality, is to add 4 multiple choice fields with only one option, each for front door, back door, patio door, window(s) and below each of those add their corresponding radio buttons.

Form Maker requires unique values for field labels, so you can add different numbers of space characters as their labels to keep them unique.
