When i click on add filed or edit a field, i am not able to do so. None of the field options show up.

Please try using another (newer) browser to open the Form Maker admin panel. This happens on the old versions of Internet Explorer.

tried different browsers and OS. Looks like it is permission issue. Can you please tell me what the setting have to be for this to work.

I'm having the same issue (latest chrome and firefox)... if this is a permissions issue and has been resolved, it would be great to post the solution here.
