Link to form from in a document

I am using forms for excursion notes for a school environment. I would like to be able to have a blank page with all the excursion note options displayed as hyperlinked text in a list (we have a lot of permission notes going home). I cant see how I can link a form to a word within a document (article). This would save creating a new menu item for each form and ending up with a large menu list.
Is that possible?

Why not create an article for each excursion note and embed the form/s.
Then create a top level article with menu link i.e. (excursion notes) that you can include links to each article/excursion note in the top level article.
Menu
- Excursion Notes (top level article)
-- Zoo Trip (sub article with embedded form)
-- School Camp (sub article with embedded form)
-- Sports Day (sub article with embedded form)
Or can you share a graphic on how you want it to look?
