I purchased the form maker for wordpress last night and i'm having trouble with an error message popping up saying that required fields aren't entered even though they are.

Help would be much appreciated, thanks. It only seems to be on the page that needs duplicate information if that helps, please see weblink for an example of the form:
(step one works fine, its the next page that is having problems)
